- Appointment Reminders & Communications. To contact you about appointments, verifications, clinic needs, and, with your consent, marketing — by phone, SMS/text, and email, including automated and AI/artificial or prerecorded voice, as described in our Privacy Policy.

- Amend. You may request that we amend information you believe is incorrect or incomplete.
- Accounting of Disclosures. You may request a list of certain disclosures we have made of your information.
- Request Restrictions. You may request restrictions on how we use or disclose your information (we are not required to agree to all requests).
- Confidential Communications. You may request that we contact you a certain way or at a certain location.
- Paper Copy. You may request a paper copy of this Notice at any time.
- Breach Notification. You have the right to be notified if a breach of your unsecured health information occurs.

We reserve the right to change this Notice and to make the revised Notice effective for information we already have as well as any information we receive in the future. The current Notice will be posted in our facility and on our website with its effective date.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
